Assessment of left ventricular remodeling and systolic synchronicity inpatients with hypertensive heart disease by RT-3DE

Abstract

Objective: Using real-time three-dimensional echocardiography(RT-3DE) to assess left ventricular remodeling in patients with hypertensive heart disease(HHD). Methods: RT-3DE data of 96 HHD(32 normal configuration, 28 concentric hypertrophy, 36 eccentric hypertrophy), 36 normal controls were acquired. Left ventricular end-diastolic volume(LVEDV), left ventricular end-diastolic epicardial volume(EDVepi) by RT-3DE and two-dimensional echocardiography(2DE) Simpson biplane method; left ventricular mass(LVM), left ventricular mass index(LVMI), left ventricular end diastolic epicardial volume index(EDVI), left ventricular remodeling index(LVRI) were calculated and compared. Result: ①LVRI, EDVI, LVMI measured by RT-3DE and 2DE were significantly different inter group, in eccentric hypertrophy groups(P<0.05), but no significantly different in normal control group, normal configuration and concentric hypertrophy(P>0.05); ②The LVRI, EDVI, LVMI had significant correlation between RT-3DE, 2DE in normal control group, normal configuration and concentric hypertrophy, the LVRI, EDVI, LVMI had no significant correlation between RT-3DE, 2DE in eccentric hypertrophy. Conclusions: RT-3DE LVRI can be a new approach for evaluating left ventricular remodeling and systolic synchronicity in clinic.
