摘要

目的：探讨CT导引下经皮心包穿刺治疗心包积液的临床价值。方法：对我院2005年4月～2006年4月因多种病因所致的15例心包积液患者行CT导引下穿刺抽吸引流并对其中12例行中心静脉引流管引流术。诊断明确后，开展针对性注药物治疗并对疗效进行评价。结果：15例心包积液病人中原发性心包恶性肿瘤2例，继发性肿瘤8例，结核性5例。介入治疗后病人短期内完全缓解11例，部分缓解2例，稳定1例。有效率100%。结论：CT导引下心包积液介入治疗疗效确切、操作精确、安全性高、并发症少，值得临床推广应用。

Abstract

Objective: To study the value of interventional therapy in pericardial effusion under CT guidance. Methods: Fifteen patients with pericardial effusion underwent CT-guided fluid aspiration and meanwhile 12 of them underwent central vein drainage tubes reservation. The final diagnosis that instructed drug treatment was got by examing the exfoliative cytology, biochemistry and tumor marker of the fluid simultaneously. Results: Among 15 cases, 2 cases had pericardial effusion derived from primarily malignant tumor of pericardium, 8 cases had secondary tumors of pericardium and 5 cases had tuberculosis of pericarditis. Eleven cases obtained complete remission, 2 partial remission and 1 stabilization. The efficacy rate was 100%. Conclusions:　The interventional therapy after CT guidance on pericardial effusion mentioned above is a safe, accurate and effective method with high success rate, less complication.
