摘要
目的：探讨胆汁淤积性肝硬化前后肠系膜上静脉、脾静脉分别对门脉灌注量的变化情况。方法：制作不全肝性胆汁淤积性肝硬化犬模型，模型成功制作12只。通过选择性插管至肠系膜上动脉和脾动脉行门脉CT灌注检查，获得正常状态时及肝硬化后正常肝叶、硬化肝叶的肠系膜上静脉灌注量（SMVP）和脾静脉灌注量（SpVP）。比较硬化前及硬化后正常肝叶、硬化肝叶间的SMVP和SpVP差异。结果：硬化肝叶的SMVP和SpVP较正常时明显减低，且SMVP的减低比例（0.59±0.16）较SpVP（0.39±0.11）明显（P=0.003）。造模后的硬化肝叶SMVP和SpVP皆较正常肝叶明显减低，且SMVP减低比例（0.56±0.17）较SpVP（0.31±0.14）明显（P=0.001）。结论：胆汁淤积性肝硬化后，SMVP和SpVP明显减低，尤以SMVP减低为著。
Abstract
To discuss the changes of superior mesenteric venous perfusion(SMVP) and splenil venous perfusion(SpVP) respectively before and after cholestatic cirrhosis by CT perfusion of portal vein in dogs. Methods: Only partial liver was involved in this cholestatic cirrhosis model. Twelve dogs survived from the process of model building. By selective catheterization in superior mesenteric artery and splenic artery by femoral Seldinger technique, SMVP and SpVP were available after portal CT perfusion. SMVP and SpVP of the normal part and cirrhotic part, before and after cirrhosis, were compared respectively. Results: SMVP and SpVP of the involved lobe decreased significantly from those of normal state, and the decrease ratio of SMVP differed significantly from that of SpVP(P=0.003). After model building, there was significant difference between SMVP and SpVP of the involved lobe and those of the normal lobe, and the decrease ratio of SMVP differed significantly from that of SpVP(P=0.001). Conclusion: After cholestatic cirrhosis, SMVP and SpVP decreased significantly, and the decrease of SMVP contributed more.
