目的:分析联合应用磁共振波谱成像(MRSI)和弥散成像(DWI)对前列腺癌(PCa)的诊断价值,探讨PCa的(Cho+Cr)/Cit比值与表观扩散系数(ADC)之间的关系。材料与方法:MRI检查使用Siemens Sonata 1.5T高场强超导MR成像仪和腹部相控阵线圈,MRSI序列采用CSI-3D-Prostate序列,采用Spectroscopy软件(Siemens Leonardo工作站)进行波谱数据处理,DWI采用EPI序列。计算PCa(n=38)、良性前列腺增生(BPH)(n=21)及正常外周带(Peripheral zone,PZ)(n=17)3种感兴趣区的(Cho+Cr)/Cit比值及ADC。对比分析(Cho+Cr)/Cit比值与ADC值的相关性。结果:PCa、BPH及PZ的(Cho+Cr)/Cit比值分别为3.98±0.12、0.51±0.19和0.38±0.09,三者代谢比率存在显著差异(P<0.05)。PCa、BPH和PZ的平均ADC值分别为(49.32±12.68)×10-5mm2/s,(86.73±26.75)×10-5mm2/s,(126.25±27.21)×10-5mm2/s,差异有统计学意义(P<0.05)。(Cho+Cr)/Cit比值与Gleason分级存在正相关性(r=0.431,P<0.05),与ADC值存在负相关性(r=-0.309,P<0.05)。结论:PCa组织代谢水平与组织微观结构密切相关,联合应用MRSI和DWI有助于提高PCa的检出率。

Abstract

To analyze the characteristics of prostate cancer(PCa) in 1H-MRSI and DWI; to explore the relationship between(Cho+Cr)/Cit ratio and ADC value. Materials and Methods: The clinical and pathologic information of 38 patients with histologically proven PCa were studied. MR spectroscopic imaging(MRSI) and diffusion weighted imaging(DWI) were performed using a 1.5T superconductive magnet scanner(Siemens Sonata) with a pelvic phased array multi-coil. MRSI was acquired using CSI-3D-Prostate sequence. All of the data was transferred to Siemens Leonardo Workstation. The (Cho+Cr)/Cit ratio of PCa(n=38), benign prostate hyperplasia(BPH)(n=21) and peripheral zone(PZ)(n=17) were calculated by spectroscopy software. DWI was carried out with a modified single-shot SE echo planar imaging(EPI) sequence. Results: The (Cho+Cr)/Cit ratio of PCa, BPH and PZ was 3.98±0.12, 0.51±0.19 and 0.38±0.09(P<0.05), respectively. The ADC value of PCa, BPH and PZ was (49.32±12.68)×10-5mm2/s, (86.73±26.75)×10-5mm2/s and (126.25±27.21)×10-5mm2/s, respectively. The (Cho+Cr)/Cit ratio and ADC value of PCa was different from that of BPH and PZ(P<0.05). (Cho+Cr)/Cit ratio correlated positively with Gleason grade(r=0.431, P<0.05), while negatively with ADC value(r=-0.309, P<0.05). Conclusion: The metabolite ratios relate closely with tissue microstructure. Combined utility of MRSI and DWI can improve the detection of PCa.
