摘要
目的：回顾分析15例经病理证实的腮腺基底细胞瘤的MSCT影像特征，探讨MSCT在腮腺基底细胞瘤中的诊断价值。材料与方法：搜集15例经手术病理证实的腮腺基底细胞瘤的MSCT检查资料，所有病例术前均进行CT平扫和双期增强扫描。评价肿瘤的平扫和增强特征，包括增强程度和方式，并与同期收集的腮腺混合瘤和腺淋巴瘤的CT增强特征进行比较；应用方差分析，比较不同病理类型肿瘤CT平扫和增强的CT值定量测量结果。结果：基底细胞瘤平扫呈不均匀的等密度结节，可见比较明显的囊变区；增强动、静脉期均显著强化，动脉期CT值增加幅度与混合瘤有显著差别，具有统计学差异（P<0.01），与腺淋巴瘤比较没有显著差别（P>0.05），静脉期CT值增加幅度与腺淋巴瘤、混合瘤比较有统计学差异（P<0.01）。结论：腮腺基底细胞瘤CT增强动脉期和静脉期均有较显著的持续性强化，伴有显著的囊变；与混合瘤和腺淋巴瘤两种常见良性肿瘤有显著差别。
Abstract
Objective: To review MSCT imaging features of 15 cases of basal cell tumor in parotid gland and explore its diagnostic value. Methods: Fifteen patients with pathologically confirmed basal cell tumor underwent preoperative MSCT plain-scan and dual-phase contrast scan. The change of CT value and enhancement patterns in the tumors were assessed. Quantitative analysis was performed to determine CT value changes of the tumors in different enhanced-phases as compared with the plain-scan. Compare these imaging features of basal cell tumor with those of other common benign tumors: pleomorphic adenomas and adenolymphomas. Analysis of variance was used for the statistical analysis. Results: Basal cell tumors showed isodense nodule with significant cystic area on CT plain-scan. At dual-phase scan, basal cell tumors showed a pattern of persistent strong enhancement with characteristic cystic areas. At arterial phase, the increase of CT value was the highest for basal cell tumors, which had statistical difference from pleomorphic adenoma(P<0.01). At venous phase, the increase of CT value was statistically different between basal cell tumor and the other two histopathologic types(P<0.05). Conclusion: Basal cell tumors showed hetergeneous isodense nodule on CT plain-scan, which showed a pattern of persistent strong enhancement with characteristic cystic areas. The CT features of basal cell tumors were different from those of the other two bengin tumors.
