摘要
目的：探讨脊髓血吸虫病的MRI表现及诊断价值。方法：对11例经临床治疗和实验室相关检查确诊或手术病理证实的脊髓血吸虫病患者的MRI资料进行回顾性分析，11例患者均行MRI平扫和增强扫描。结果：11例均显示远端脊髓和圆锥肿胀，T1WI呈等或稍低信号，T2WI 9例呈不均匀高信号，信号强度低于脑脊液，2例显示硬膜下结节伴脊髓信号异常。增强后11例中9例可见髓内病灶呈多种形态强化，其中7例呈多发小结节或小斑片状强化，2例呈肿块样强化。11例中6例显示脊髓表面线样强化，5例可见神经根强化，2例硬膜下结节轻度强化。结论：脊髓血吸虫病的MRI表现有一定特征，结合疫水接触史、免疫学检查及诊断性治疗有助于明确诊断。
Abstract
Objective: To discuss the MRI findings of spinal cord schistosomiasis and its diagnostic value. Materials and Methods: To review the MRI data of 11 cases of spinal cord schistosomiasis, all cases were diagnosed by the trial treatment and laboratory test or by the surgical-pathological examination, all cases were examined with MRI plain scans and contrast scans. Results: MRI showed mild to moderate expansion of the distal spinal cord and conus in 11 cases, the lesions were iso- to slightly hypo-intense relative to cord tissues on T1- and heterogeneous hyperintense on T2-weighted spin-echo images, the signal intensity was lower than cerebrospinal fluid(CSF) on T2WI, soft tissue mass at intradural-extramedullary with abnormal signal intensity of the distal spinal cord in 2 cases. And intramedullary multiple small nodular or patchy enhancement in 7 cases, irregular masslike enhancement in 2 cases, peripheral linear enhancement on the cord surface in 6 cases and enhancement of the cauda equina roots in 5 cases and mild enhancement of the intradural mass in 2 cases after intravenous administration of gadolinium-DTPA. Conclusion: The MRI of spinal cord schistosomiasis presents some features, combined with the history of exposure to schistosoma-contaminated water, the immunological studies and the trial treatment are helpful to the diagnosis of the disease.
