摘要
目的:评价产前超声预测胎儿肺重量-体重比(Fetal lung to body weight ratio,FLB)的准确性,并初步探讨该超声指标在诊断胎儿肺发育不良(Pulmonary hypoplasia,PH)中的应用价值。方法:对33例拟引产胎儿于引产前1~3d行超声检查,得到其超声预测的FLB值,其中胎儿体重通过二维生长指标经Hadlock公式生成,胎肺重量应用三维超声体积自动测量技术(VOCAL)计算的胎肺容积转换得出。引产后6h内对引产胎儿进行尸检,比较尸检测得的胎儿实际FLB值与超声预测值,并评价超声预测FLB对PH的诊断价值。结果:超声与尸检得到的FLB的平均值分别为0.017±0.009、0.019±0.007,差异无统计学意义,以尸检FLB值为金标准,超声预测FLB值的偏倚及95%一致性界限分别为-0.002及-0.007~0.004;超声预测FLB诊断胎儿PH的敏感性、特异性、阳性预测值、阴性预测值、诊断准确率分别为83.3%、90.4%、83.3%、90.5%及87.9%。结论:超声能较准确的预测胎儿FLB,其对产前评价和诊断PH具有一定的临床意义。
