摘要

目的：探讨磁共振扩散加权成像（DWI）和表观扩散系数（ADC）在膀胱病变鉴别诊断中的应用价值。方法：以53例膀胱癌及12例良性膀胱病变作为病例组，11例正常膀胱壁作为对照组。分析病例组及对照组的DWI并测量病变及正常膀胱壁的ADC值。结果：所有53例膀胱癌表现为扩散受限。12例良性病变中，2例轻度扩散受限，余10例无扩散受限。DWI上检出膀胱癌的敏感度为100%，特异度为83.3%，准确度为96.9%。膀胱癌的平均ADC值为（0.76±0.17）×10-3 mm2/s，显著低于良性病变的（1.21±0.20）×10-3 mm2/s，并显著低于对照组的（1.55±0.15）×10-3 mm2/s（F=89.04，P<0.001）。结论：DWI和ADC在膀胱良恶性病变的鉴别诊断中具有较好的应用价值。

Abstract

Objective: To investigate the value of MR diffusion-weighted imaging(DWI) and apparent diffusion coefficient(ADC) in the differential diagnosis of urinary bladder lesions. Methods: Fifty-three patients with urinary bladder cancer and 12 patients with benign urinary bladder lesions constituted the study group and 11 individuals with normal urinary bladder constituted the control group. DWI of all individuals were analyzed and ADC values of lesions and normal walls were measured. Results: All the 53 urinary bladder cancers displayed a restriction in diffusion. Two of 12 the benign lesions displayed a mild restriction in diffusion and the others displayed no restriciton. The sensitivity, specificity and accuracy of DWI in the diagnosis of malignant urinary bladder cancer was 100%, 83.3%, and 96.9% respectively. The mean ADC value of the urinary bladder cancer was (0.76±0.17)×10-3 mm2/s, which was significantly lower than that of the benign lesions(1.21±0.20)×10-3 mm2/s and the control group(1.55±0.15)×10-3 mm2/s(F=89.04, P<0.001). Conclusion: DWI and ADC have a high value in the differential diagnosis of benign and malignant urinary bladder lesions.
