摘要
目的：探讨混合型完全性肺静脉异位连接（TAPVC）的解剖类型、诊断方法及要点，以及超声心动图对本病的诊断价值。方法：近5年在我中心确诊为混合型TAPVC的患者，分析其主要的影像学检查结果。结果：17例混合型TAPVC患者有14例为心上+心内型，3例为心上+心下型。超声心动图发现5例肺静脉回流梗阻，而CT仅发现1例肺静脉回流梗阻。超声心动图准确探查到所有肺静脉回流径路者有7例，准确探查到3条者有4例。结论：混合型TAPVC以心上+心内型最多见。结合超声心动图和多排螺旋CT扫描是术前评估的最佳策略。超声心动图能准确显示大部分肺静脉回流径路，对显示肺静脉回流梗阻优于多排螺旋CT。
Abstract
Objective: To study the anatomy variants and diagnostic methods for the mixed total anomalous pulmonary venous connection(TAPVC), and the value of echocardiography as a diagnostic tool for this disease. Methods: To retrospectively analyze the main imaging data for those who were diagnosed as mixed TAPVC in our tertiary center in recent 5 years. Results: Among the 17 mixed TAPVC patients, 14 were supracardiac plus cardiac, 3 were supracardiac plus infracardiac. Pulmonary venous obstruction as observed in 5 of them, all by echocardiography, and only 1 by multi-detector CT scan. Echocardiography detected all pulmonary veins precisely in 7 patients, 3 pulmonary veins correctly but 1 incorrectly in 4 patients. Conclusion: Supracardiac plus cardiac are the most common type of mixed TAPVC. The combination of echocardiography and multi-detector CT scan are the best strategy for preoperative evaluation of these patients.
