摘要
目的:采用Meta分析的方法研究钆塞酸二钠(Gd-EOB-DTPA)增强MR评价肝纤维化分级的诊断准确性。方法:计算机检索2004—2012年国内外公开发表的Gd-EOB-DTPA增强MR评价肝纤维化分级的诊断准确性文章,采用METAVIR计分系统标准进行分级,检索时间均由2004年1月—2012年12月。两名评价员按照纳入排除标准独立选择文献并根据QUADAS(Quality assessment of diagnosic accuracy studies)评价条目标准进行质量评价。使用Revman软件和Meta-Disc软件进行Meta分析,绘制森林图及SROC曲线。结果:共检索到相关文献264篇,最终纳入6个研究,共564例受试者。Meta分析结果示各组间存在统计学异质性,采用随机效应模型进行合并。Gd-EOB-DTPA增强MR对轻度肝纤维化(≥F1)组、明显肝纤维化(≥F2)组、严重肝纤维化(≥F3)组、早期肝硬化(F4)组的合并敏感度和特异度分别为0.92和0.81、0.76和0.86、0.85和0.95、0.87和0.92。SROC曲线下面积分别为0.847 8、0.932 2、0.983 7、0.932 6。结论:Gd-EOB-DTPA增强MR对肝纤维化分级具有较高的诊断效能,尤其是对于明显肝纤维化(≥F2)组、严重肝纤维化(≥F3)组及早期肝硬化(F4)组。
Abstract
Objective: To evaluate the accuracy and quality of Gd-EOB-DTPA-enhanced MR imaging for the diagnosis of staging hepatic fibrosis. Methods: We searched the Chinese Database, Cochrane Library, Pubmed, Embase, Web of science. The retrieval times were from 2004. 1 to 2012. 12. Diagnostic tests of Gd-EOB-DTPA-enhanced MR imaging for the diagnosis of hepatic fibrosis stage were included. Data were extracted, and the quality of included studies was evaluated according to the QUADAS(Quality assessment of diagnosic accuracy studies) items. The heterogeneity test and the SROC(Summary receiver operating characteristc) curve and Meta-analysis were performed by Meta-Disc and Revman. Result: A total of 264 relevant articles were retrieved and 6 studies were included in our review involving 564 participants. Meta-analysis showed that the heterogeneities among studies were high, the pooled sensitivity and specificity of ≥F1, ≥F2, ≥F3, F4 hepatic fibrosis stages were 0.92 and 0.81, 0.76 and 0.86, 0.85 and 0.95, 0.87 and 0.92, respectively. The AUCs of SROC were 0.847 8, 0.932 2, 0.983 7 and 0.932 6, respectively. Conclusion: Gd-EOB-DTPA-enhanced MR imaging has a high value for staging hepatic fibrosis ≥F2, ≥F3, F4.
