摘要
目的:探讨弥散加权成像(DWI)与磁共振动态增强(DCE-MRI)在前列腺癌分期诊断中的应用价值。材料与方法:回顾性分析48例经病理证实为前列腺癌的患者的DWI及DCE-MRI影像学资料,并与病理结果和临床分期对照。结果:48例前列腺癌中DWI结合DCE-MRI诊断≤B期11例、C期23例、D期14例,分期准确性为85.4%,≤B期、C期、D期的表观扩散系数(ADC)值分别为(0.85±0.15)×10-3 mm2/s、(0.73±0.13)×10-3 mm2/s和(0.64±0.09)×10-3 mm2/s,经统计学分析,三期差异有统计学意义(F=8.06,P<0.05)。Spearman相关分析表明癌区ADC值与肿瘤分期呈负相关(r=-0.53,P<0.01)。结论:DWI结合DCE-MRI对前列腺癌分期的准确性较高,可以用于前列腺癌的分期。
Abstract
Objective: To explore the diagnostic value of diffusion weighted imaging(DWI) and magnetic resonance dynamic contrast-enhanced imaging(DCE-MRI) in the staging of prostate cancer. Materials and Methods: DWI and DCE-MRI data of 48 cases of pathologically proved prostate cancer was retrospectively analyzed, and correlated with pathology and tumor staging. Results: Of the 48 cases, DWI and DCE-MRI diagnosed 11 cases of stage ≤B, 23 cases of stage C and 14 cases of stage D. The accuracy of tumor staging was 85.4%. The mean apparent diffusion coefficient(ADC) values of the tumor regions were (0.85±0.15)×10-3 mm2/s, (0.73±0.13)×10-3 mm2/s and (0.64±0.09)×10-3 mm2/s, respectively. There was statistical significance among the three stages(F=8.06, P<0.05). Spearman correlation analysis showed the ADC values in cancerous regions were negatively correlated with tumor staging(r=-0.53, P<0.01). Conclusions: The accuracy of ADC and DCE-MRI is high in staging of prostate cancer, and ADC combined with DCE-MRI might be useful to the staging of prostate cancer.
