摘要

目的：通过对下颌骨标本进行螺旋CT扫描重建及测量，并与标本的实际测量值进行比较，建立螺旋CT在下颌骨测量方法的规范，进而更精确的指导牙种植手术操作。方法：利用螺旋CT（Siemens Somatom Spirit）对30例下颌骨标本进行编号扫描，将获得的数据信息进行多平面图像重建，在直接获得的矢状面重建图像和冠状面重建图像上测量，然后对重建指示线做以调整，再在重建指示线调整后矢状面重建图像和冠状面重建图像上测量，利用统计学方法对获得的CT测量数据与标本实际测量数据进行一致性比较，分析测量方法对数据准确性的影响。结果：通过分析30例下颌骨标本CT扫描测量数据与标本实测数据，在进行多平面图像重建测量时，未做重建指示线调整的图像测量数据误差较大，而调整重建指示线后的测量数据则与颌骨标本实测数据一致性较高。结论：牙种植前利用螺旋CT进行多平面图像重建测量时，在颌骨矢状面重建图像和冠状面重建图像上对重建指示线做以调整，能够获得比较准确的测量数据，可用于牙周炎的治疗观察和避免种植失败和损伤下颌神经提供可靠依据。

Abstract

Objective: To establish the normative measuring method on reconstructed CT images of mandible for guiding the dental implant surgical procedure more accurately, the mandibular specimens were measured on reconstructed CT images and compared with the actual data of the specimens. Methods: Spiral CT(Siemens Somatom Spirit) was performed on 30 cases of mandible specimens which were numbered for scan, the normal coronal and sagittal images were reconstructed for measurement of some set line in molar region. Then the adjusted coronal and sagittal images were reconstructed according to the morphological structure of each specimen, and the same set line was measured again. The two series data obtained on two series of reconstructed CT images were compared with the actual data of mandibular specimens. Results: The data get from the adjusted coronal and sagittal reconstructed images were more accurate than that get from the normal coronal and sagittal reconstructed images. Conclusions: The adjusted coronal and sagittal reconstructed images were more useful for measurement of the set line on preoperative CT images in molar region to guide the dental implant surgical procedure.
