摘要
目的：探讨定量组织速度成像及组织追踪成像技术评价心肌梗死患者左心室局部心肌的收缩功能。方法：获取标准心尖位左室长轴观、二腔观和四腔观，分别应用定量组织速度成像和组织追踪成像离线分析20例正常人和33例心肌梗死患者左室各节段长轴方向的心肌速度曲线和位移曲线，计算收缩期峰值速度（Vs）及收缩期各取样点向心尖方向位移（Ds），用二维超声心动图测量左室射血分数（LVEF）。结果：心肌梗死组左室各心肌节段Vs及Ds比正常对照组显著降低；心肌各水平节段平均Vs（mVs）与平均Ds（mDs）较正常人明显下降。用QTVI及TTI测量的左心室基底段及中间段的mVs及mDs与用二维超声心动图测量的LVEF显著相关，而心尖段的平均mVs及mDs与LVEF无显著相关。结论：QTVI及TTI能准确判断心肌纵向收缩速度和幅度，是无创评价局部心肌收缩功能定量精确的方法。
Abstract
Objective: To quantitatively evaluate regional systolic myocardium function of left ventricle(LV) in patients with myocardial infarction by means of quantitative tissue velocity imaging(QTVI) and tissue tracking imaging(TTI). Methods: Twenty healthy subjects and 33 patients with myocardial infarction were studied by QTVI and TTI. Off-line LV regional velocity and displacement profiles along LV apical long-axis view, apical two-chamber view and four-chamber view were obtained synchronously, and peak velocities of LV regional myocardium during systole(Vs), and peak systolic displacement(Ds) were measured. LV ejection fraction(LVEF) was determined by 2D echocardiography using Simpson’s rule. Results: Both Vs and Ds of LV segments in myocardial infarction group(MI group) were significantly lower than those in normal control group(control group). Mean Vs(mVs) and Ds(mDs) of different segment level in MI group were lower than those of the control group. mVs and mDs in basal and middle segments level related significantly and in apical segments level did not relate to LVEF. Conclusions: The QTVI and TTI can judge the longitudinal systolic velocities and amplitudes, and is a non-invasive and quantitative method to assess systolic function of left ventricular regional myocardium.
